Notice of Disclosures

Following is a list of the required disclosures all students, and employees, (prospective and employed) are entitled to receive from an institution of higher education on an annual basis. All items marked with an asterisk (*) are available upon request. All others are distributed through various publications.

PROCEDURES TO REQUEST INFORMATION: To request information pertaining to the following list, see the Admissions Director, Mrs. Richards, or the Support Services Director, Mrs. Campbell. In requesting information regarding the school’s state licensing and accreditation status, see Mrs. Marias, the Director.

1. *Financial Assistance Available: Federal and Institutional financial aid available at this school—Consumer Information and the School’s Catalog.

2. *Institution’s Satisfactory Academic Progress Policy: Satisfactory Progress Policy

3. *Institutional Information: Information on the Cost of Attendance, including tuition & fees, books, & supplies, typical room & board, transportation costs, and any additional costs involved in the program—Consumer Information & School Catalog.

4. *Institutional Refund Policy: Any student withdrawing, or terminated, from the institution, and who is not a recipient of the Title IV grants or loan aid, will be eligible for the Institution Refund calculation—Refund Example Sheet.

5. *Procedures and Requirement for Officially Withdrawing from School: The Academy’s Policy and Procedure to officially withdraw from the program of study—Consumer Information.

6. *Return of Title IV Grant or Loan Assistance: The treatment of Title IV funds when a student withdraws from an institution and is a recipient of Title IV aid—Refund Example Sheet.

7. *Program Information: Program requirements to enroll, graduate (diploma requirements), description of the clinic, classroom and facilities, and names of the school’s faculty—School Catalogs & Standards for Training.

8. *Names of the Accrediting Agency and State Licensing Agency: Names and addresses of accrediting agency and state licensing board—School Catalog.

9. *Description of Special Facilities and Services Available to Disabled Students—School Catalog.

10. *Student-Right-To-Know: The Institution’s Completion and Graduation Rates is provided to all prospective students as an enclosure with the catalog, along with this Notice of Disclosure Memo. It is also available on our website www.creativehair.com and updated by July 1 of each year. To acquire a hard copy, please see the Support Services Director or Admissions Director.

11. Annual Security Policies and Crime Statistics—Each year by October 1, enrolled students, employees, as well as prospective students and employees, will receive an updated list of any crimes that occurred on campus, and on public property, which is adjacent to the school, during the three most recent calendar years. In addition, they will receive the Academy’s “Security Policies and Crime Statistics”, which includes information on the reporting of any criminal activity and how to help maintain a safe environment—School Security Policies and Crime Statistics Chart. (for the applicable years)

12. *Plans for Improving the Academic Programs—When the Academy deems it necessary to begin utilizing a revised textbook, the textbook may be viewed in the Admissions Office, as well as any curriculum changes, if applicable. Should curriculum changes occur, these will also be reflected within the Course Content Outline for each program affected.

13. *Copyrighted Materials Policies & Sanctions—The school’s policy and related sanctions in the unauthorized distribution of copyrighted material, including peer-to-peer file sharing, may subject a student to civil and criminal liabilities—Available for review in the Support Services, Admissions, and Director’s Offices.
